When are the contribution reports due?
Contribution reports and payment are due by the 10th of the month following the month in which hours are worked. Reports are considered late if received after the 20th of the following month, and late charges will be assessed.
What are the late charges for delinquent reports?
Liquidated damages are assessed at 10% of the Health, Pension & Training unpaid contributions, or $50.00, whichever is greater. Interest is assessed at 8%.
Where do I mail my contribution reporting form and payment?

Can I submit several months on a single report?
No. Employee hours and wages must be submitted on separate reports for each work month. Work months cannot be combined into a single reporting form. For questions, call (626) 279-3072 or email lu831ermaint@pswadmin.com.
What if there is a rate change mid-week?
If there is a rate change mid-week, you may submit two employer report forms. Indicate "Supplemental Hours" on the supplemental report (old rates) and send it with the "New Rates" report. For assistance, call (626) 279-3072 or email lu831ermaint@pswadmin.com
Who do I request a “Status/Release” letter from?

Do I have to use the Fund’s contribution reports or can I use my own worksheets when remitting contributions?
The Trust Fund prefers that employers use the monthly contribution reports provided. If using your own worksheet, employees should be listed alphabetically with Social Security numbers, and hours and wages should be shown next to each name. Employers may also recreate the monthly reporting form for their needs.
Can I remit one check for more than one Fund?
Yes. One check may be submitted for one contribution report or multiple reports covering all funds.
What if I cannot pay my contributions on time?
Submit the original contribution report without payment, along with a letter explaining why payment cannot be made and when payment is expected. The Trust Fund will contact you regarding the contributions. However, notifying the Trust Fund does not waive any applicable liquidated damages or interest charges.
Why am I charged liquidated damages?
Under the Collective Bargaining Agreement, reports are considered late if not received by the 20th of the month and are subject to applicable late charges.
Why is the charge so huge for being one day late?
Liquidated damages are assessed the same whether payment is one day late or thirty-one days late. The assessment is based on unpaid contributions as provided in the Collective Bargaining Agreement.
Do I have to mail my contribution reports and payment or can they be hand-delivered?

A receipt will be issued for hand-delivered payments. Payments must be made by check, money order, or cashier's check. Cash and credit cards are not accepted. Overnight mail cannot be sent to the P.O. Box address.

What happens if I become delinquent with contributions?
The Trust Fund will send letters and make phone calls to determine the reason for the delinquency and arrange payment. If the matter cannot be resolved, it may be referred to legal counsel for collection. Under ERISA, employers may be liable for attorneys' fees, costs, interest, and liquidated damages.
Is there a grace period for submitting payment?
Yes. Employer reports are due on the 10th of the month but are not considered delinquent if received by the 20th, providing a 10-day grace period.
